How To: Complete the BC COVID-19 Vaccination Attestation Form (Windows, Mac, Mobile)

Summary

This KB article goes over the steps how to navigate and upload your proof of vaccination document on the Bellevue College Vaccination Attestation website or how to submit a vaccine exemption request. 

PLEASE NOTE: If you are receiving a look-up error while logging into the form that says, "1. Lookup - Lookup returned no record and aborted connector execution," please see our attestation FAQ page.


Fully Vaccinated Submissions

  1. Navigate to the Bellevue College Vaccination Attestation website
  2. Sign in using your BC email and password
  3. Verify that your identity information is correct and accurate
  4. Select your Employee Status. If you are an employee (Including student-employees) select “Yes” if you are a student only and not employed by the college, select, “No.”
  5. Select the Vaccination Status of 'I am fully vaccinated' (note: if you have only received one shot of a two dose series or you have not received a single vaccine shot, you will need to complete your vaccination series before continuing. Please return and resubmit this form once you have both doses. For vaccine exemption requests, please see below.)
  6. Agree to the Vaccination Confirmation by selecting the "Yes, I Understand and Confirm that I am Fully Vaccinated" check box
  7. Select your Vaccine Version and enter the date of receiving the dose(s)
  8. Select “Yes” If you have received one or more booster doses and enter the Vaccine Version and enter the date of receiving the dose(s). If you have not had a booster, Select “No.”
  9. Upload your Proof of Vaccination document by selecting the Choose File button (note: you can take a picture of your document with a smartphone/tablet or scan a copy to your computer):
    • Smartphone/Tablet - Select your photo library, select the photo of the document and then select/choose to upload the photo
    • Windows, MAC - In the window that appears navigate to the location of the documentation you wish to upload (i.e in your Photos Library, Documents, Downloads, Desktop etc), select the document, then select to open/upload the document
  10. Click the Submit button

Vaccine Exemption Submissions

  1. Navigate to the Bellevue College Vaccination Attestation website
  2. Sign in using your BC email and password
  3. Verify that your identity information is correct and accurate
    Select your Employee Status. If you are an employee (Including student-employees) select “Yes” if you are a student only and not employed by the college, select, “No.”
  4. Select the Vaccination Status of 'I have not received a vaccine'
  5. Select Yes for Exemption Request (note: if you select 'No' you will need to acknowledge that if you are not fully vaccinated by October 18, 2021 and you have not received a medical or religious exemption, Employees with Bellevue College may be terminated. For students, you may not be able to enroll into classes.)
  6. Select Medical or Religious for your exemption reasoning:
    • Medical Reasons: 
      • Employees - You must download the Medical Exemption Request Form, complete the form with the assistance of your medical provider and provide the completed form to Human Resources
    • Religious Reasons: 
      • Employees - Please download the Religious Exemption Request Form, complete it, and upload it using the Choose File option on the form and navigate to where you saved the completed form (i.e. in your Documents, Downloads, Desktop etc.)
  7. Check the checkbox 'I affirm that if I receive an exemption that I will follow all state, local and college guidelines for unvaccinated individuals'
  8. Click the Submit button

By submitting this form you declare and confirm that the information provided is accurate and true and may be subject to verification by Bellevue College, the state, and local public health officials, including observing my CDC vaccination card, state immunization information system record, or other documentation.
